When you do contact the Pension Benefit Guaranty Corporation it is
important to have as much information as possible that can help them
track down your benefit. You will want to provide them with your full
name, address, phone number, social security number, date of birth and
the name and location of the employer in question. Additionally, if you
have any documents from the plan itself be sure to include the
nine-digit Employer Identification Number (EIN) and the three-digit
plan identification number.
